Carried Oleg Makarov, Vasili Lazarev for rendezvous with Salyut 4; but during second-third stage seperation third stage failed to separate from second stage but still ignited. The crew demanded that the abort procedures be implemented but ground control could not see the launch vehicle gyrations in their telemetry. Soyuz finally was separated from by ground control command at 192 km, and following a 20.6+ G reentry, the capsule landed in the Altai mountains, tumbled down a mountainside, and snagged in some bushes just short of a precipice. The crew was worried that they may have landed in China and would face internment, but after an hour sitting in the cold next to the capsule, they were discovered by locals speaking Russian. Total flight duration was 1574 km and flight time 21 minutes 27 seconds. Lazarev suffered internal injuries from the high-G reentry and tumble down the mountain side and never flew again. Both cosmonauts were denied their 3000 ruble spaceflight bonus pay and had to apeal all the way to Brezhnev before being paid.

Intending to spend 60d on Salyut 4, Vasily Lazarev & Oleg Makarov survived a high altitude aborted launch OTD 1975. In minutes, they experienced up to 21.3 g, landed on a snow-covered slope (-7 C), sliding towards a 152m sheer drop, stopped by the parachutes snagging on vegetation

Cosmonauts escape a close call launch
Last update: April 9, 2025

(...) On April 6, 1975, or around 24 hours after the accident, the Soviet TASS news agency issued a statement with an unprecedented admission of the Soyuz launch failure. It said that during the third-stage operation, the rocket had deviated from its prescribed path triggering an automated separation of the spacecraft for the return to Earth. The statement also disclosed that the Descent Module had made a soft landing near the town of Gorno-Altaysk in Western Siberia and that cosmonauts Vasily Lazarev and Oleg Makarov were returned to the cosmodrome in good health. (50)

It is not exactly clear why the Soviet authorities decided to make the fact of the accident public, since a suborbital ballistic flight could easily be concealed unlike any orbital mission which would almost universally be tracked by foreign radar after its first full orbit. According to one hypothesis, Soviet officials were concerned that during ongoing preparations for the joint Apollo-Soyuz mission, involving multiple contacts between the American and Soviet space officials, rumors could start circulating, souring the atmosphere around the upcoming historic flight. A complete denial of the accident could lead to a lack of trust between the two sides especially when it involved the performance of a system directly involved in the joint flight. (...)
